On some occasions, a Nurse Care Manager my email you to request a change to a care pathway or the creation of a new care pathway. If you approve the request, complete the following steps to create a new pathway.

On the Navigation panel, select the Care Management toggle, and select Settings.

Select
Care Pathways.

Select New or select a care pathway to modify.

Enter the Name and Description of the pathway.

Select
Save.
After you finish: Share the changes made to the care pathway with the Nurse Case Manager team, by sending an email to the NCM team, or reviewing as an update at a standing NCM team meeting.
